Daily Operational Information
Coaches will not allow a gymnast, who is under 18, to leave a session under different circumstances than indicated here unless we have written confirmation regarding changes to these statements. It is also important that we are contacted if a gymnast is not going to attend a session.
The following club policies will be followed at all times: Safeguarding Policy, Codes of Conduct and Disclaimer Form. They can be found at https://www.spartatc.co.uk/documents/
I agree to emergency medical treatment or first aid, which, in the opinion of a qualified medical practitioner or first aider is considered necessary. I also understand that should such a situation arise; all reasonable steps will be taken to contact the parent or an alternative emergency contact.
Photography and Videography. I/We give permission for photos or videos to be taken and used for training or publicity purposes by Sparta Trampoline Club SCIO. If you wish to opt out of this please contact the club directly.
I/We agree to adhere to all policies issued by the club.
I/We have read and understood all the above information and agree to it all. If I am a gymnast that is under 18 my parent / guardian has also read the above and also agrees.
I/We understand that Trampolining is a high risk sport that whilst everything will be done to prevent injury during classes, accidents can occur.
